What to expect at your first visit:

Your initial contact with our office will involve answering some basic questions so we can better serve your needs.  Dr. Banning will review your health history form and dental experiences.  She will then complete an exam of your teeth and your “bite”.  If she thinks it is necessary, an x-ray will be taken for diagnosing purposes.  If you or your child are ready to proceed with treatment, the treatment plan will be discussed as well as all fees.  This visit is complementary, please feel free to call for an appointment.

Patient Forms:

If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.
